Проблема диагностики и лечения пациентов с черепно-мозговой травмой является важной социальной проблемой ввиду высокого уровня инвалидизации и смертности трудоспособного населения.

ЦЕЛЬ ИССЛЕДОВАНИЯ

ЦЕЛЬ ИССЛЕДОВАНИЯ: провести ретроспективный анализ пациентов с травматическими внутричерепными гематомами, находившихся на лечении в нейрохирургических отделениях г. Ульяновска.

МАТЕРИАЛЫ И МЕТОДЫ

МАТЕРИАЛЫ И МЕТОДЫ. Проведен анализ 2567 историй болезни пациентов, 76 актов судебно-медицинских вскрытий и 354 протоколов операций. Пациентам проводилась КТ головы, неврологический осмотр с оценкой уровня сознания по шкале ком Глазго.

РЕЗУЛЬТАТЫ

РЕЗУЛЬТАТЫ. Из 2567 человек травматические внутричерепные гематомы диагностированы у 306 (11,9 %) пациентов: из них 71,2 % мужчин. Основной причиной черепно — мозговой травмы стал бытовой травматизм — 57,8 % случаев, затем криминальная травма — 29,1 % случаев, производственная травма — 3,3 % случаев, ДТП — 9,8 % случаев. В большинстве случаев наблюдались субдуральные гематомы, составившие 39,5 % случаев, затем эпидуральные — 19,9 % случаев и внутримозговые гематомы — 21,6 % случаев. По временному фактору чаще встречались острые гематомы — 75,4 % затем расположились подострые — 14,4 %, хронические — 6,9 %, прогрессирующие — 3,3 %. На момент госпитализации в ясном сознании находилось 48,4 % пациентов, в умеренном оглушении — 12,1 % пациентов, в глубоком оглушении — 14,1 % пациентов, в сопоре — 9,8 % пациентов, в умеренной коме — 10,1 % пациент, в глубокой коме — 3,9 % пациентов, в терминальной коме — 1,6 % пациентов. Летальность у пациентов с травматическими внутричерепными гематомами составила 19,9 %.

ЗАКЛЮЧЕНИЕ

ЗАКЛЮЧЕНИЕ: проведенный анализ лечения пациентов с травматическими внутричерепными гематомами показывает актуальность проблемы ввиду частой травматизации пациентов трудоспособного возраста. В структуре внутричерепных гематом, выделен особый вид гематом — прогрессирующие гематомы, требующие контроля в диагностике и лечении. Проблема высокой летальности пациентов с ЧМТ сопровождающейся наличием внутричерепных гематом в Ульяновской области является актуальной и требует продолжения изучения для минимизации основных качественных показателей при оказании нейротравматологической помощи пострадавшим.

MATERIALS AND METHODS: The analysis of 2567 case histories of patients, 76 acts of forensic autopsy and 354 protocols of operations was carried out. Patients underwent CT scan of the head, neurological examination with an assessment of the level of consciousness according to the Glasgow Coma Scale.

RESULTS

RESULTS. Of 2567 people, traumatic intracranial hematomas were diagnosed in 306 (11.9 %) patients: 71.2 % of them were men. According to the time factor, acute hematomas were more common — 75.4 %, then subacute — 14.4 %, chronic — 6.9 %, progressive — 3.3 %. At the time of hospitalization, 48.4 % of patients were in clear consciousness, 12.1 % of patients were in moderate stunning, 14.1 % of patients were in deep stunning, 9.8 % of patients were in stupor, and 10.1 % were in moderate coma. patient, in deep coma — 3.9 % of patients, in terminal coma — 1.6 % of patients. mortality in patients with traumatic intracranial hematomas was 19.9 %.

CONCLUSION

CONCLUSION: the analysis of the treatment of patients with traumatic intracranial hematomas shows the relevance of the problem due to the frequent traumatization of patients of working age. In the structure of intracranial hematomas, a special type of hematoma has been identified — progressive hematomas that require control in diagnosis and treatment. The problem of high mortality in patients with TBI accompanied by the presence of intracranial hematomas in the Ulyanovsk region is relevant and requires further study to minimize the main quality indicators in the provision of neurotraumatological care to patients.
