Патогенез травматического ангиоспазма включает в себя возникновение вазоконстрикции сосудов головного мозга в результате реакции на продукты распада гемоглобина (воспалительная реакция в ответ на субарахноидальное кровоизлияние (САК)), приводящей к отеку мозга, ишемии и внутричерепной гипертензии, что может повлиять на тактику лечения. Однако на данный момент времени нет общепринятых рекомендаций по поводу лечения данного осложнения.</p><sec><title>ЦЕЛЬ ИССЛЕДОВАНИЯ</title><p>ЦЕЛЬ ИССЛЕДОВАНИЯ: оценка необходимости проведения транскраниального дуплексного сканирования (ТКДС) в предоперационном периоде у пострадавших с черепно-мозговой травмой для определения оптимальной тактики хирургического лечения.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ: Проведен анализ диагностики и проведенного лечения 34 пострадавших с ЧМТ, находившихся в НИИ СП им. Н. В. Склифосовского. Всем пациентам были проведены исследования — КТ головного мозга и транскраниальное дуплексное сканирование (ТКДС). Оперативное вмешательство было выполнено 9 пациентам. Пострадавшие, которым проводилось оперативное лечение, были разделены на 2 группы: У 3 из 9 больных ТКДС была выполнена до операции (группа 1), у 6 из 9 пациентов ТКДС была выполнена в послеоперационном периоде (группа 2).</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ: У 15 из 34 (44 %) выявлен ангиоспазм по данным ТКДС. На ТКДС, выполненной до операции, ангиоспазм был обнаружен из 3 человек у 2. У пациентов, которым выполняли ТКДС в послеоперационном периоде, ангиоспазм выявлен у 3 из 6 больных. 10 пациентам оперативное вмешательство не проводилось. Больным из группы 1, было проведено: 1 — декомпрессивная трепанация черепа (ДКТЧ), 2 — костно-пластическая трепанация черепа (КПТЧ) (1 в последующем потребовалось повторное вмешательство с выполнением ДКТЧ в связи с развившимся отеком-ишемией на фоне ангиоспазма). К больным, из группы 2, была применена хирургическая тактика: 2 — ДКТЧ, 4 — КПТЧ (2 в последующем потребовалось повторное вмешательство — ДКТЧ в связи с развившимся отеком-ишемией на фоне ангиоспазма). В послеоперационном периоде летальных исходов 4: 3 — после превентивно проведенной ДКТЧ, 1 — после КПТЧ с повторным вмешательством в виде ДКТЧ в связи с развившимся отеком-ишемией на фоне ангиоспазма.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ: Ангиоспазм является значимым фактором развития отека и ишемии головного мозга. Необходимо проведение мониторирования всем пострадавшим с ЧМТ, с целью улучшения исхода заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><p>Posttraumatic angiospasm is one of the complications of traumatic brain injury. The pathogenesis of traumatic angiospasm includes the occurrence of vasoconstriction of cerebral vessels as a result of a reaction to hemoglobin breakdown products (an inﬂammatory reaction in response to subarachnoid hemorrhage (SAH)), leading to cerebral edema, ischemia and intracranial hypertension, which may affect treatment tactics. however, at the moment there are no generally accepted recommendations for the treatment of this complication.</p><sec><title>THE PURPOSE OF THE STUDY</title><p>THE PURPOSE OF THE STUDY: to assess the need for transcranial duplex scanning (TCDS) in the preoperative period in patients with traumatic brain injury to determine the optimal tactics of surgical treatment.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: An analysis of the diagnosis and treatment of 34 victims with TBI who were at the N.V. Sklifosovsky Research Institute for Emergency medicine was carried out. All patients underwent brain CT and transcranial duplex scanning (TCDS). Surgery was performed in 9 patients. The victims who underwent surgical treatment were divided into 2 groups: In 3 out of 9 patients, TCDS was performed before surgery (group 1), in 6 out of 9 patients, TCDS was performed in the postoperative period (group 2).</p></sec><sec><title>RESULTS</title><p>RESULTS: Angiospasm was detected in 15 out of 34 (44 %) according to TCDS data. Angiospasm was detected in 2 out of 3 people on TCS performed before surgery. In patients who underwent TCS in the postoperative period, angiospasm was detected in 3 out of 6 patients. Surgery was not performed on 10 patients. Patients from group 1 underwent: 1 — decompressive cranial trepanation (DCTCH), 2 — bone-plastic cranial trepanation (CRTCH) (1 subsequently, repeated intervention with DCTCh was required due to the developed edema-ischemia on the background of angiospasm). Surgical tactics were applied to patients from group 2: 2 — DCTCH, 4 — CPTCH (2 subsequently required repeated intervention — DCTCH due to the developed edema-ischemia on the background of angiospasm). In the postoperative period, deaths were 4: 3 — after preventive CT, 1 — after CT with repeated intervention in the form of DCT due to the developed edemaischemia on the background of angiospasm.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: Angiospasm is a signiﬁcant factor in the development of edema and ischemia of the brain.
