В последнее десятилетие наметилось новое направление в малоинвазивной нейрохирургии – трансорбитальная нейроэндоскопическая хирургия (TONES).</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Изучение анатомо-хирургических параметров латерального трансциалиарного трансорбитального доступа и результатов его применения.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. На 9 замороженных препаратах головы человека с двух сторон выполнено моделирование латерального трансцилиарного трансорбитального доступа с оценкой его анатомо-хирургических характеристик на различных этапах. С использованием доступа оперированы 3 пациента.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. По данным исследования, угол атаки на интракраниальном этапе по сравнению с внутриглазничным этапом значительно уменьшается с комфортного для хирургических манипуляций 16–25° до минимально возможного для работы в 5–10°. Зона доступности также уменьшается более чем в 2 раза.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Латеральный трансорбитальный доступ обеспечивает адекватные по анатомо-хирургическим параметрам условия для проведения хирургических манипуляций в пределах глазницы, ее верхней и латеральной стенок и прилежащих отделов твердой мозговой оболочки.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. In the last decade, a new direction in minimally invasive neurosurgery has emerged – transorbital neuroendoscopic surgery (TONES).</p></sec><sec><title>AIM</title><p>AIM. Study of the anatomical and surgical parameters of the lateral transtialial transorbital approach and the results of its use.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. Modeling of the lateral transciliary transorbital approach was performed on 9 frozen specimens of the human head on both sides, with an assessment of its anatomic-surgical characteristics at various stages. Three patients were operated on using this approach.</p></sec><sec><title>RESULTS</title><p>RESULTS. According to the study, the angle of attack at the intracranial stage, compared to the intraorbital stage, is significantly reduced from 16–25°, comfortable for surgical manipulations, to the minimum possible for work, 5–10°. The area of surgical exposure is also reduced by more than 2 times.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION.
