Артериовенозные мальформации (АВМ) головного мозга являются достаточно редкой сосудистой патологией, которая обладает крайне негативным воздействием не только на качество жизни пациентов, но и высокими рисками грубой инвалидизации и летального исхода при их разрыве. В лечении таких мальформаций в последние годы все больше внимания уделяется внутрисосудистому подходу, в том числе в качестве единственного метода терапии с целью тотальной эмболизации патологического клубка сосудов, формирующего мальформацию, с использованием неадгезивных композиций. Однако клинические исходы и эффективность данного метода не всегда носят удовлетворительный характер и остаются предметом дискуссий и требуют дальнейшего изучения для выявления факторов риска и методов снижения частоты интраи послеоперационных осложнений.</p></sec><sec><title>ЦЕЛЬ</title><p>ЦЕЛЬ. Оценка частоты нарастания и структуры очагового неврологического дефицита в ближайшем послеоперационном периоде, а также функциональных исходов (mRS) у пациентов после тотальной эмболизации артериовенозной мальформации головного мозга неадгезивными композициями.</p></sec><sec><title>МАТЕРИАЛЫ И МЕТОДЫ</title><p>МАТЕРИАЛЫ И МЕТОДЫ. Ретроспективный анализ результатов лечения 48 пациентов, перенесших тотальную эмболизацию АВМ неадгезивными композициями (2023–2025). Оценивали неврологический статус до и после операции, данные нейровизуализации после операции (компьютерная томография, магнитно-резонансная томография), функциональный статус (mRS) до операции и при выписке из стационара.</p></sec><sec><title>РЕЗУЛЬТАТЫ</title><p>РЕЗУЛЬТАТЫ. В раннем послеоперационном периоде нарастание очаговой неврологической симптоматики наблюдается у 45,8 % пациентов, причем в 72,7 % этих наблюдений по данным нейровизуализации выявляется появление очагов острой ишемии головного мозга. Функционально благоприятный исход (mRS≤2) достигнут у 79,2 % пациентов, что свидетельствует о перспективности метода при оптимизации техники вмешательства.</p></sec><sec><title>ЗАКЛЮЧЕНИЕ</title><p>ЗАКЛЮЧЕНИЕ. Ишемические поражения головного мозга являются одной из основных причин развития неврологического дефицита после тотальной эмболизации АВМ неадгезивными композициями. Несмотря на высокую частоту нарастания очаговой симптоматики, функционально благоприятный исход достижим у большинства пациентов при условии оптимизации техники эмболизации и послеоперационного ведения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>INTRODUCTION</title><p>INTRODUCTION. Cerebral arteriovenous malformations (AVMs) are a relatively rare vascular pathology that has an extremely negative impact not only on patients’ quality of life but also carries a high risk of severe disability and death in the event of rupture. In recent years, the treatment of these malformations has increasingly focused on intravascular approaches, including as a sole method of therapy for total embolization of the pathological vascular tangle forming the malformation using non-adhesive composites. However, the clinical outcomes and effectiveness of this method are not always satisfactory and remain a subject of debate. Further study is required to identify risk factors and methods for reducing the incidence of intraand postoperative complications.</p></sec><sec><title>AIM</title><p>AIM. To evaluate the incidence and pattern of focal neurological deficits in the immediate postoperative period, as well as functional outcomes (mRS) in patients following total embolization of cerebral AVMs using non-adhesive composites.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. A retrospective analysis of treatment outcomes in 48 patients who underwent total AVM embolization with non-adhesive composites (2023–2025). Preand postoperative neurological status, postoperative neuroimaging data (CT, MRI), and functional status (mRS) before surgery and at hospital discharge were assessed.</p></sec><sec><title>RESULTS</title><p>RESULTS. In the early postoperative period, worsening focal neurological symptoms was observed in 45.8 % of patients, with neuroimaging data revealing foci of acute cerebral ischemia in 72.7 % of these cases. A functionally favorable outcome (mRS≤2) was achieved in 79.2 % of patients, demonstrating the potential of this method when the intervention technique is optimized.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. Ischemic brain damage is one of the main causes of neurological deficit after total AVM embolization with non-adhesive composites.
