Embolization of the tumor vascular network as the first stage in the surgical treatment of intracranial meningiomas (literature review and clinical case)

Abstract

   Meningioma is a tumor of the meninges of the brain and spinal cord, arising primarily from meningothelial cells of the arachnoid mater. Among all primary brain tumors in adults, meningioma ranks second in frequency after glial tumors, accounting for 18–35 % of cases.

   The main treatment for meningiomas is surgical.

   However, removal of meningiomasis often complicated by their marked vascularization, which can result in significant intraoperative blood loss or reduced radicality of surgical treatment.    To address this problem, a combined treatment approach has been studied, involving endovascular embolization of the vascular network supplying the meningioma as the first stage, followed by a microsurgical stage of operative treatment.

   Aim – yo analyze the scientific literature and assess the effectiveness of tumor vascular network embolization as the first stage of treatment, and to present our own clinical case to confirm the effectiveness of this treatment method.

   A search and analysis of the scientific literature were performed using the scientific electronic libraries eLIBRARY and PubMed. A literature review describing the problem was carried out, and a clinical case is presented. Performing superselective embolization of the meningioma vascular network as the first stage of surgical treatment is an effective method for reducing intraoperative risks of blood loss and increasing the degree of radical tumor removal, especially for large meningiomas and highly vascularized meningiomas.
