Quality of life in patients with glioblastoma

Abstract

INTRODUCTION. Because patients with glioblastomas have a limited life expectancy and suffer from neurological, cognitive, and systemic symptoms caused by both the tumor itself and the treatment, an approach to assessing quality of life and its impact on survival is crucial for these patients.
AIM. To determine the differences in the quality of life of patients with glioblastomas after surgical treatment based on the QLQ-BN20 questionnaire, depending on the resection class according to the RANO-resect classification (2022), and their impact on survival.
MATERIALS AND METHODS. The study included 63 patients with IDH-wildtype glioblastomas. The RANO criteria (2022) were used to assess radicality. The QLQ-BN20 questionnaire was used to assess the quality of life before discharge, 1, 3, and 6 months after surgery.
RESULTS. The questionnaire indicators were analyzed depending on the resection class – motor disorders (p=0.039; p=0.019; p=0.009; p=0.005) and uncertainty about the future (p=0.013; p=0.013; p=0.023; p=0.030) differed at all stages of follow-up. When assessing epileptic seizures, differences were found after 6 months (p=0.021). At the second stage, an analysis of the impact of indicators on survival was carried out. Predictors of worsening survival were increased uncertainty about the future before discharge (p<0.001), after 1 (p<0.001), 3 (p<0.001) and 6 months (p< 0.001), motor disorders before discharge (p=0.023) and after 3 months (p=0.014), difficulties with communication before discharge (p=0.023) and after 1 month (p=0.008), weakness in the legs after 6 months (p=0.041) and drowsiness before discharge (p=0.007) and after 3 months (p=0.003).
CONCLUSION. Quality of life assessment using the QLQ-BN20 questionnaire is a useful tool for predicting outcomes in patients with glioblastomas that go beyond traditional oncological criteria.
